  • This part-time position will provide housing expertise and support for the I Choose Home NJ/Money Follows the Person program.
  • The I Choose Home NJ/MFP program is a federally funded partnership between the NJ Client and the NJ Office of the Long-Term Care Ombudsman designed to help individuals living in nursing homes move back into the community with the services and supports they need.
The housing program will:
  • Assist nursing home residents (and their social workers, managed care case managers, and other entities.) to identically accessible and affordable housing options in the community.
  • Meet with housing stakeholders statewide to increase affordable housing options for the I Choose home/MFP population, including landlords, housing developers, non-profit groups, realtors etc.
  • Provide housing education and guidance to managed care organizations (MCO's) housing specialists (who will manage this work when I choose home NJ ends), including developing and hosting quarterly housing webinars, fielding individual's questions about housing and connecting MCO's to available housing resources.
  • Attend internal and external meetings and events related to NJ housing policy/issues.
  • Track progress of I choose Home candidates/participants seeking housing, including maintaining the list of the programs NED-2 vouchers.
Experince Required:
  • Applicants must have at least 2 years' experience connecting vulnerable populations (e.g. low-income, homeless, elderly, people with disabilities, formerly incarcerated, etc.) with housing resources. S/He must be familiar with low-income housing entities and resources throughout the state-voucher programs, public housing authorities, etc. and comfortable working at both individual and policy levels.
