Title: Speech-Language Pathologist (SLP) – Home Health
Location: San Jose, CA
Duration: 26 weeks
Shift: Day 5x8-Hour (08:00 – 17:00)
Job Responsibilities and Required/Preferred Skills & Requirements:

Provide speech-language pathology services to patients in a home health setting, with a primary focus on geriatric populations.

Evaluate, treat, and manage patients with communication, cognitive, and swallowing disorders in the home environment.

Develop and implement individualized plans of care focused on functional outcomes and patient-centered goals.

Complete timely and accurate documentation using the Kinnser/WellSky charting system; all charting must be completed in the clinic.

Maintain a minimum of 23–24 patient visits per week to qualify for a full-time hour guarantee.

Collaborate with interdisciplinary care teams to support comprehensive patient care.

Demonstrate strong time management, organization, and independence while traveling within assigned territory.
Required Qualifications:

Active state Speech-Language Pathologist license (must be in hand at time of submission).

BLS certification.

Minimum 1 year of home health experience and 2 years of total clinical experience as an SLP.

Ability to work independently and manage patient schedules effectively.
Preferred Skills & Experience:

Experience with geriatric home health patients and wound care environments.

Strong documentation skills and familiarity with home health workflows.

Self-starter with a patient-focused approach and strong follow-through on charting.
