Title: Speech-Language Pathologist (SLP) – Home Health
Location: San Jose, CA
Duration: 26 weeks
Shift: Day 5x8-Hour (08:00 – 17:00)

Job Responsibilities and Required/Preferred Skills & Requirements:

• Provide speech-language pathology services to patients in a home health setting, with a primary focus on geriatric populations.

• Evaluate, treat, and manage patients with communication, cognitive, and swallowing disorders in the home environment.

• Develop and implement individualized plans of care focused on functional outcomes and patient-centered goals.

• Complete timely and accurate documentation using the Kinnser/WellSky charting system; all charting must be completed in the clinic.

• Maintain a minimum of 23–24 patient visits per week to qualify for a full-time hour guarantee.

• Collaborate with interdisciplinary care teams to support comprehensive patient care.

• Demonstrate strong time management, organization, and independence while traveling within assigned territory.

Required Qualifications:

• Active state Speech-Language Pathologist license (must be in hand at time of submission).

• BLS certification.

• Minimum 1 year of home health experience and 2 years of total clinical experience as an SLP.

• Ability to work independently and manage patient schedules effectively.

Preferred Skills & Experience:

• Experience with geriatric home health patients and wound care environments.

• Strong documentation skills and familiarity with home health workflows.

• Self-starter with a patient-focused approach and strong follow-through on charting.
