NYCCC-Queens Campus is seeking services of a valid licensed Speech-language Pathologist for

children/adolescents. Services to be provided shall include, but may not be limited to, patient evaluations,

treatment planning, speech therapy, consultation to staff, and related duties.

• Documentation of evaluations and therapy must be completed per the Uniform Case Record and NYCCC

policies.

• Service hours shall not exceed 15-20 hours per week with a maximum of 750 hours annually.

• Facility General hours are 8:30am to 5:00 pm, Monday – Friday.

-Scheduling is flexible and can be arranged as per the mutual needs of the facility and Speech Therapist.

• HIPAA/HITECH and other mandated training must be completed as determined by OMH/Facility.

• Speech Therapist must visibly wear facility issued badge/identification at all times during course of the

work schedule.

• Speech Therapist is expected to integrate readily into the treatment team and interact with staff at all

levels in their professional capacity.

The Speech- Language Pathologist must readily provide a copy of a valid license to practice in the State of

New York and will need to provide updates accordingly upon the request of the facility. The Speech Therapist

shall adhere to all requirements, regulations, and ethical practices, standards and licensing requirements. The

Speech-Language pathologist should be a NYS teacher of Speech and Hearing Handicapped.

Speech- language pathologist should have direct clinical experience working with children/adolescents.

The selected vendor would be required to provide resumes along with the mandatory forms prior to

interviewing as required by external NYS agencies as well as proof of immunizations including

PPD/Tuberculosis testing, and the flu vaccine.
