Title - Registered Nurse
Location - York Street Clinic, St, Utica NY 13502 Duration - 06 + months (may be extended) Shift - 8am-4:30pm Job description Duties include, but are not limited to: • Provide Injections for clinic clients and complete necessary documentation; make medication sheet for each client that gets injections • Complete clinical documentation that meets compliance standards, medical necessity guidelines, and agency standards as to timeliness and accuracy of documentation • Perform home/community visits as needed • For client's on Clozaril: o Track required monthly lab work on spreadsheet o Submit lab work into the REMS system o If clients are overdue, reach out o Fax lab work to pharmacies if requested • Ensure EKGs are done as required • Complete Metabolics on clinic clients and complete necessary documentation; make any referrals and linkages that are needed • Complete Health screens on clinic clients and complete necessary documentation • Maintain spreadsheets for injections/health screens • Triage client calls • Review RHIO hospital notes and brief relative staff • Answer calls from the Medline and process requests/handle issues with Scripts, side effects, etc. • Coordinate/advocate care with medical PCP's and specialties o Call doctor's offices and lab sites to get lab results, medical notes o Call hospitals to get reports on the clients that have gone to the ER or are in-patient • Obtain Prior authorizations, and handle insurance issues regarding medications, (mostly injections) • Maintain AOT binder • Assist prescribers with various tasks as needed Minimum qualifications: Possession of a license and current registration as a registered professional nurse in New York State. | ||||||
