Job description
Facility is looking for a physical therapist needed to provide on-site physical therapy assessment and treatment for inpatient at Hutchings Psychiatric Center (HPC). Requests for service will be arranged through the HPC Medical Clinic. Candidate will work under the direction and guidance of the HPC Medical Clinic and other HPC Staff. Need is for 48 hours per month of service.
 
Services will include, but are not limited to:
  • Comprehensive assessment and evaluation of problem, including the interpretation of tests and measurements, to determine intervention and assist in the diagnosis and prognosis.
  • Plan for service, including specific goals and the interventions. If actions are delegated to another licensed professional, specify those tasks and how the patient's progress will be assessed or reviewed. If plan is modified this should be noted along with recommendations for follow up or other intervention.
  • Execute interventions.
  • Documentation of above as well as all interventions.
  • Discharge summary, including specific notation of any plans for future interventions.
  • •Supply HPC with the information needed to assure that the services provided meet the applicable standards of The Joint Commission, which accredits HPC. This information includes evidence of primary source verification of required credentials, evidence that the provider has a current New York State license to practice physical therapy services and evidence of initial and continuing competence as part of a performance evaluation system.
Minimum qualifications:  Licensed New York State Physical Therapist and/or Masters or Doctorate in Physical Therapy from an accredited program.
