Registered Nurse Medicine /Surgical

GENERAL JOB SUMMARY:
Coordinate and deliver patient care utilizing the Nursing Process and following all established standards of practice within State and Federal guidelines. Provide supervision of non-RN personnel and act as a resource for ancillary staff on assigned unit. Supervise unit activities when assigned to charge or clinical coordinator role. Act as preceptor to new staff and/or students as requested. Adhere to all Hospital policies and procedures, including the Code of Conduct and Corporate Compliance Program. Provide safe and therapeutic care using the values inherent in our hospital culture.

  1. Provides direct patient care to a select group of patients and advocates on their behalf
  2. Assesses patient's physical and psychosocial condition throughout the continuum of care.
  3. Develops and communicate patient plan of care reflecting patient issues and proposed interventions to achieve desired patient outcome.
  4. Collaborates with patient, family and healthcare team in implementing plan of care
  5. Provides education as indicated after assessing the learning needs of the patient and family
  6. Communicates effectively with all members of the health care team and related departments to provide accurate information about the care of the patients.
  7. Formulates and maintains effective working relationships with peer, managers and other team members.
  8. Displays a positive attitude and works to achieve an environment of respect, accountability, teamwork, integrity, and compassion. Does not support or take part in disruptive or negative behavior.

  10. Practices the 5 rights to patient safety when administering medications, blood or blood products and treatments. (right patient, right medication, right route, right dose, right time)
  11. Responds to crisis in a calm, efficient manner, directing others concisely.
  12. Evaluates patient outcomes, effectiveness of plan of care and readiness for discharge and/or continuing care. Revise plan of care as needed and appropriately refer for additional services where indicated.
  13. Documents pertinent information, observations and all aspects of patient care clearly, concisely and in a timely manner.
  14. Maintains equipment and utilizes supplies in a safe, cost effective manner
  15. Participates in and supports performance improvement and risk management activities.
  16. Participates in clinical research activities and demonstrates evidence base practice.
  17. Participates in ongoing regulatory readiness compliance.
  18. Supports hospital-wide and unit specific initiatives (e.g. Planetree, Magnet, Stroke Certification, Joint Certification, etc.)
  19. Participates in the development and implementation of goals, objectives, policies and procedures and standards of care, which contribute to the achievement of the organization's strategic plan
  20. Completes required continuous training and education, including department specific requirements
  21. Takes responsibility for own professional development.
  22. Encourages nurses to actively seek ways to enhance professional knowledge and competence through participation in professional nursing organizations
  23. Complies with hospital attendance policy and department staffing policy and rotations.
  24. Adheres to hospital and department specific dress code policy.
  25. Complies with hospital and departmental policies and procedures and adheres to external agency requirements
  26. Performs other related duties as assigned or requested in order to maintain a high level of service.
UNIT SPECIFIC COMPETENCIES & REQUIREMENTS:
  1. I) Receives specialty training for CAPD
  2. Acts as a resource to other units as the CAPD expert
  3. Provides direct patient care for hemo dialysis patient
  4. Participates in the awareness programs (Wound Wednesday, Fall Friday & Adopt a Room)
  5. Cross trains the Psychiatry unit
  6. Attends multidisciplinary rounds
  7. Orients to charge/clinical coordinator role
  8. Participates in daily discharge rounds
QUALIFICATIONS REQUIRED:
  1. Graduate of an accredited School of Nursing.
  2. Valid license as Registered Nurse in Connecticut.
  3. BSN strongly preferred.
  4. BLS certification required.
  5. Maintain minimum of 6 contact hours annually.
