Additional Information or Terms: For Home Health traveler: mileage reimbursement rate for business travel to .535 per mileall Tele, ICU and Neuro (neurosciences and neurosurgery) travelers are required to take the NIH Stroke Scale. It can be taken for free at www.apexinnovations.com2017 Orientation Dates: January 3 (Tuesday), 16, 30February 13, 28 (Tuesday)March 13, 27April 10, 24May 8, 22June 6 (Tuesday), 19July 5 (Wednesday), 17, 31August 14, 28September 12 (Tuesday), 25October 9, 24 (Tuesday)November 6, 20December 4, 18** Floating Policy** Agency’s traveler shall commit to adhere to Facilities’s floating policies: To float within assigned module, to float between facilities, that is between Renown Regional Medical Center and Renown S. Meadows Medical Center; and to float between modules at both facilities. Floating is based on appropriate competences and orientation to unit, if appropriate. So in order to speed up the process on orientation day for travelers. Occupational health would like all travelers to fill out the ORIENTATION FORM and it must be completed prior to their first day. In addition, please let them know that on the respiratory questionnaire they are answering yes or no to the first 3 pages if they have a lot of no’s then they can put no and draw a line down so they don’t have to keep writing it. On the 4th page they will put the number of any yes’ and an explanation and then on the 5th page it’s just the top part they need to fill out. Lastly, a lot of travelers are showing up without their BLS, and ACLS cards can you please ask the agencies to have their travelers bring these cards just on their first day please? STAFFING AGENCY MUST BE JCAHO ACCREDITED in order to submit to RN positions. STAFFING AGENCY's JCAHO program will support the standards set for utilizing outside services within RENOWN HEALTH. Renown will accept either a 2 step TB test taken within a year or one TB Quantiferon and yes we will accept either the titers or vaccines. *************PLEASE TAKE NOTE!!! THE TUESDAY PRIOR to commencement of assignment, evidence of health status to include annual health assessment, two-step TB clearance, immunizations and titers, as outlined by Renown Health, OCC Health Dept. If complete Supplemental Staff health documentation is not received by RENOWN HEALTH, from STAFFING AGENCY, prior to Supplemental Staff arrival at RENOWN HEALTH, RENOWN HEALTH may provide the necessary health services. Should Renown Health provide Supplemental Staff with services, such as immunizations, x-ray and/or health assessment, the STAFFING AGENCY will be assessed a $500.00 flat fee, to cover necessary health requirement services performed by RENOWN HEALTH. If the traveler start date is postponed, due to a delay in the receipt of the traveler’s health docs, the $500.00 penalty fee will also apply. By submission of your candidate to this or any job posting, you are agreeing to these terms and penalties.Each Assignee must complete drug testing at Renown Health, upon arrival on day one of orientation, prior to orienting. Failure to comply with drug testing and /or positive drug tests will result in immediate termination of the assignment (See II. L. Below). Staffing Personnel discovered in breach of this provision are subject to termination for cause RENOWN HEALTH will not consider for assignment any Staffing Personnel that:• has worked as permanent staff at a health care facility in Washoe County within the past 12 months • is currently a permanent resident within a sixty-mile radius of Washoe County• has worked at RENOWN HEALTH with another agency within the past 30 days * This facility does not pay extra for on call. If your candidate works call that needs to be paid out of the regular bill rate. We will honor a 2hr call back min if they are called in to work.
