The purpose of positions in this classification is to provide paraprofessional patient care to hospice patients, as established in the patient plan of care, and under the supervision of a registered nurse. Job Responsibilities: This listing is not to be construed as all-inclusive. Other duties may be required and assigned. • Provides paraprofessional nursing care within the residential hospice house or travels to patient homes to provide care within the established plan of care. Continually monitors the patient’s condition; answers call lights, anticipates patient and family needs and makes rounds on a regular basis. Takes vital signs. Takes and records temperature, pulse and respiration. Assists with patient positioning, patient ambulation, transfers, patient cleaning, toileting activities, catheter care, etc. • Documents care in accordance with established procedures. Observes patient condition and records and reports changes in patient’s condition. Communicates changes in the patient’s condition, needs, and any concerns to the registered nurse or Hospice Program Director. • Aides with activities of daily living including assisting with bathing, dressing, toileting and grooming including shaving, skin care, hair care, nail care, and oral hygiene needs. Changes bed linens. • Interacts with patients, family members and others. Listens to patients and families questions and concerns and responds in a timely manner. Communicates any patient concerns to the patient care team and Hospice Program Director. • Provides water, snacks, and meals as appropriate. Assists with feeding patients as necessary. • Performs light housekeeping such as vacuuming, cleaning and dishes. Performs laundry tasks. • Provides caregiver relief. Sits with and provides comfort and support to patients. Reads to patients. • Greets visitors, patients, family members and others that come into the hospice house. Provides general information over the telephone and in-person. • Completes daily assignment sheets and timesheets to supervisor on a timely basis which accurately document completion of assignments, hours worked, mileage, travel and other reimbursable and non-reimbursable time worked. • Attends and participates in mandatory meetings, in-services, and conferences. • Performs room cleaning after patient death or discharge in accordance with housekeeping and infection control standards. • Attends safety training sessions and ensures for compliance with safety rules and procedures. Assists in ensuring for patient safety. Identifies and notifies the Hospice Program Director of potential safety hazards. Reports any work-related accidents and injuries in a timely manner. Complies with the procedures for documenting and submitting leave and light duty time.
