Summary/Objective
Under general supervision, individual is responsible for assisting participants with educational, social, recreational resources and employment opportunities in the community.

Essential Functions/Job Duties

  • Assist participants to explore educational, social, recreational resources and employment opportunities in the community utilizing agency vehicles and/or public/alternative transportation.
  • Facilitate and provide discovery, training, support, inclusion and accommodations for participants in the community and at work.
  • Independently engage persons served in structured and unstructured community activities and volunteer opportunities through use of agency vehicles and/or public/alternative transportation.
  • Facilitate/write meaningful community-based outcomes that align with each participant's person-centered plan.
  • Develop employment plans in collaboration with Employment Services.
  • Deliver services which adhere to CMS, DODD and CARF rules and standards.
  • Ensure compliance with federal, state and local rules and statues.
  • Maintain an environment free of health and safety hazards.
  • Establish, maintain and promote excellent working relationships among participants, community sites and employers.
  • Develop partnerships with community resources utilizing networking skills and knowledge of the Columbus metropolitan area using agency vehicles and/or public/alternative transportation.
  • Uphold the rights, dignity and respect of all persons served as well as promote independence and self-advocacy skills.
  • Teach new skills in community settings (both hard and soft skills).
  • Provide assistance with meals, personal care and transport individuals to/from community activities and job sites.
  • Maintain records and prepare reports that are compliant with Medicaid billing requirements via the use of applications.
  • Monitor progress and prepare reports detailing activities, progress and future services.
  • Participate in all required meetings and staff training.
 
Education and Experience
  • High School Diploma or equivalent; Bachelor's Degree preferred.
  • One-year experience in related field is preferred.
  • Must be able to provide nursing delegation as directed.
  • Must have strong communication, verbal and written skills.
  • Must demonstrate exceptional training and organizational skills.
  • Knowledge of the city's resources and strong research skills preferred.
  • Must have the ability to represent participants and agency in a professional manner.
  • Must be familiar with disability-related issues, services/resources, accommodations and assistive technology.
  • Must complete eight hours of approved DODD training.

Required Licensure and Certifications
  • Adult Services Worker Certification.
  • Must hold valid client or equivalent first aid and cardiopulmonary resuscitation certification.
  • Possess and maintains a valid State of Ohio Driver's License; meet and maintain insurability standards to drive for company business.
 
Work Environment
This position operates in a professional office environment. This role routinely uses standard office equipment.

Physical Demands
The physical demands described are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of the job, the employee is required to stand; walk; sit; use hands to finger, handle, or feel objects, tools or controls; reach with hands and arms; climb stairs; talk and hear. Individual must be able to lift, bend, push and pull up to 50 pounds.

Position Type/Expected Hours of Work
This is a full-time position. Individual must be able to work a flexible schedule including evenings and weekends.
