IMMEDIATE NEED! HIRING LEADER NEEDS TO INTERVIEW ASAP. CANDIDATE WILL BE BASED IN THE LIVINGSTON, NJ OFFICE BUT WILL TRAVEL TO BERGEN, MORRIS, AND SOMERSET COUNTIES.

POSSIBLE TEMP TO HIRE OPPORTUNITY BASED ON PERFORMANCE

A Registered Professional Nurse is responsible for managing a client's care, developing plans of care, and acting as a care manager and coordinator for the client. This position responsibilities include, but are not limited to:

- Performs home care assessments and re-assessments for clients and their environment to determine home health care needs
- In conjunction with the family and physician and/or authorized practitioner and supervisor, develops and implements the plan of care based on the assessment
- Obtains authorized practitioners orders
- Notifies authorized practitioner of changes in the client's condition
- Provides skilled nursing care as outlined in the plan of treatment to include the following: nursing services, treatments and preventative procedures including those requiring substantial specialized skill ordered by the physician and authorized practitioner
- Evaluates the effectiveness of his/her nursing service to the individual family
- Observes signs and symptoms and changes in the client's physical or emotional condition and consults with the physician or authorized practitioner concerning alteration of the plan of treatment in consultation with the supervisor, as necessary
- Reports all patient changes and information to the supervisor in a timely manner
- Submits a record of visits/hours worked each day using santrax
- Participates in case management conferences with other team members
- Teaches, supervises and counsels the client and caregivers regarding the nursing care needs and other related problems in the clients home
- Documents services rendered, changes noted in the clients condition and/or family and home situation and evaluates client response to treatment
- Attends in service and continuing education
- Participates in CQI activities
- Develops the paraprofessional care plan, orients paraprofessional to care of plan and supervises according to agency policy, including instruction, demonstrations, and return demonstration as necessary. Supervises the aide in the special needs of the client and documents supervision according to the agency policies and procedures
- Observes the client rights and confidentiality

QUALIFICATIONS:

- Graduate of an approved school of professional nursing
- Current licensure and registration to practice nursing for the state in which they are working
- One year of Med/Surg experience
- One year of experience in eldercare field
