Job SummaryReports directly to the Director of Social Service. Medical Social Worker assignment includes primary responsibility for coverage of Acute Care Cases, Outpatient, and Behavioral Health areas. Responsible for provision of social work intervention for cases assigned; customizes care plans based on individual patient care need timely and efficiently. Social Work focuses on the social determinants of health that may be an issue; assist with dc planning activity as indicated; care plans based on individual patient care need timely and efficiently. Responsible for information and referral services; provides counseling, supportive services and post discharge follow up as indicated. Works with patient and families navigating through the healthcare system promoting advocacy and education; included but not limited to data collection, statistics on caseload activity and reporting weekly and monthly reports to the director timely and efficiently; support groups, outpatient inquiries etc and other duties as assigned by the Director.Job RequirementsMinimum EducationBS Degree or MSW in SW or related fieldMinimum Work ExperienceMin. 1-3 yrs. experience in hospital setting, SNF, or health/community agencyMinimum Licenses and CertificationsSocial Workers MA State SW licensure-LCSW, LSW, LSWA and /or LICSWRequired SkillsAbility to work independently and with a teamExcellent verbal and written communication skills requiredComputer experience for data collection, report writing, and quality monitoringFlexibility is a mustFunctional DemandsPhysical Requirements:Exerts up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects. Frequently reaches (extending hands and arms in any direction), and handles (seizing, holding, grasping, turning, or working with hands).Organizational ExpectationsBehavioral Attributes:The following behavioral attributes are required: achievement motivation, concern for order, flexibility, initiative, self-confidence, customer service oriented, interpersonal effectiveness, teamwork, analytical thinking and information seeking.Essential FunctionsCustomizes and completes a specific care plan based on the assessment of patient needs within 48 working hours of being assigned the case.Completes documentation using the documentation standards noted by department policies and procedures, state and federal regulations.Completes clear and concise documentation noting patient and family participation, multidisciplinary involvement, and other planning information as required by the department, state and federal regulation agencies.Provides Utilization Review work as needed in a timely and efficient manner and documenting process in Medical Record for each intervention. Keeping director informed of issues.Case closings: casework statistics are to be completed and submitted to department secretary within 5 days post dc. Stat sheet to be completed in full.Keeps abreast of department policies and procedures, as well as, state and federal regulation guidelines, (i.e. JCAHO, DMH, DPH, etc.) to ensure compliance with department policies and procedures, state and federal regulations.Meets the patient social service needs which includes, meeting with patients, and family when appropriate, to provide supportive services through emotional support and reassurance and psychosocial intervention.Provides information and education to patients and their families regarding the care plan as part of their specific care needs and works closely with members of the multidisciplinary team including, physicians, patients, families, hospital staff and community agencies.Maintains a safe environment for patients by assuring that sharp objects, toxic substances and other objects potentially harmful to children are not accessible.Informs patients of their patient rights when indicated (i.e. discharge planning, guardianship, court commitments, admission/hospitalization,Power of Attorney, Interpreter Services etc).Provides services to upon request, for community outreach ie walk-ins, telephone inquiries, other functions etc.Assures that the environment of care or services is free of objects which may contribute to the likelihood of patient falls, slips or trips. Makes readily available assistive devices for ambulation. Complies with policies and procedures relating to gait belts and similar devices.Is aware of the increased risk of falls, slips, and trips for elderly patients and visitors, and assures that the environment of care or services is free of objects and spills which may contribute to the likelihood of patient falls, slips, or trips.Is sensitive to the tendency of older patients to have hearing difficulties.Verifies that he or she is speaking with appropriate level of loudness so that the patient hears instructions, questions, and conversations with caregiver, but that offense is not given to the patient as the result of the voice being excessively loud. Is able to differentiate problems with hearing versus problems with comprehension.Correctly interprets verbal and nonverbal response pain management treatments and when appropriate, informs nursing and/or physician about observation.Takes appropriate actions to assure physical security of children and complies with policies and procedures related to the prevention of neonate, infant and child abduction.Is aware of reduced endurance and strength present in many elderly, and provides accommodation for seating during extended interviews, discussions with secretaries, receptionists and other clerical personnel, or when the patient is waiting for transportation.Statement of Other DutiesThis document describes the major duties and responsibilities for this job, and is not intended to be a complete list of all tasks and functions.
