As a Home Health PT you will evaluate mobility, strength and current level of function, while setting goals to improve balance, walking, transfers, as well as patient education. Unlike a rehabilitation setting or even an outpatient clinic, unique emphasis is placed on safe mobility in and around the patient’s residence. PT’s will also make recommendations on equipment and provide training on proper use of equipment. Door to Door visits are required.

Basic Duties
The Staff Physical Therapist, the Physical Therapist (PT), reports to the Clinical Supervisor, Physical Therapy and provides both direct and indirect patient care services. The PT works primarily with adult and geriatric patients from diverse socioeconomic and cultural backgrounds, although pediatric and adolescent may also be treated.
Under general direction and upon receipt of written referrals, the PT plans and administers a full range of physical therapy services for patients with a variety of physical and cognitive impairments and disabilities. Services include patient evaluations using physical therapy tests and measurements, establishment of therapeutic intervention goals and development of therapeutic intervention programs, and implementation of therapeutic intervention plans utilizing physical therapy modalities and procedures.
The Staff Physical Therapist maintains all applicable patient records related to the delivery of patient care services. While performing job duties, the PT may also supervise support personnel including Registered Physical Therapist Assistants and Rehabilitation Aides as well as PT and P.T.A. students. The Staff PT actively participates in departmental and organizational programs, including performance improvement activities, clinical education programs, and in services and staff development activities.
