For over 12 years our specially-trained team of physicians, nurse practitioners and physician assistants have provided in-home medical care to patients who have difficulty getting to the doctor's office.
Besides serving individual patients, our client provides clinical care support services for office-based physicians, Medical Groups, Health Plans, Hospitals, and other Health Care Organizations

Nurse Practitioner / Physicians Assistant

Our Nurse Practitioner is responsible for providing primary patient care to our home bound patient population. In this hands-on role, you will be the face of quality care for the organization. Our Nurse Practitioner will collaborate with their assigned Physician and our internal Clinical teams to deliver outstanding primary care to our patients in their private homes, Board and Care facilities and Assisted Living Facilities

  1. Travel from patient to patient by car to perform initial visits, physical examinations, perform preventative health measures and perform regularly scheduled patient visits
  2. Order, interpret and evaluate diagnostic testing to identify and assess the patients clinical concerns and healthcare needs
  3. Coordinate care plans with Collaborating Physician and Clinical Services team to provide comprehensive patient care plans
  4. Prescribe or recommend medications or treatments for chronic or therapeutic treatment plans
  5. Ensures patient satisfaction or develop and implement steps for improvement
  6. Escalate questions and concerns to Collaborating Physician or Medical Director regarding clinical and operational guidelines to ensure effective patient care, safety and clinical outcomes
  7. Maintain standards of coding and details of encounter data for completeness, accuracy, and timeliness to ensure optimal reimbursement levels
  8. Participate when necessary, in the mentorship and training of new Advanced Practitioners
  9. Enhance knowledge and professional development through participation in educational programs, literature reviews, in-services and workshops
  10. Participate in department meetings and trainings, external meetings with vendors and partners and present a high level of professionalism and confidence in the organization
  11. Participate in the On-Call schedule including patient documentation
  12. Actively participate in the promotion of the practice and the organization

  • Graduate of an accredited college or university with an advanced degree in Nursing
  • Current Nurse Practitioner or Physician Assistant license with no license restrictions in the state to practice
  • Current, unrestricted DEA license
  • 1 year previous experience in a house call, visiting nurse practitioner or similar environment preferred along with 3 years in a clinical setting
  • Fluency in Spanish Highly Desired
  • Strong working knowledge of an EMR system; E-Clinical Works or similar product strongly desired
  • Understanding of HCC coding and risk adjustment methodology highly desired
  • Strong communication skills, especially with the geriatric community and their families
